Overview
Spencer Clarke Group are working closely with Liverpool City Council to appoint a Social Care Assessor to their team. The role is on an agency contract which is highly likely to be extended on an ongoing basis. Rate of Pay - £15 - £19 per hour. Negotiable depending on experience. Location – Liverpool, L15 2HE. The Main Purpose of the role: To undertake needs assessments, develop care plans and support plans which underpin social care intervention to individuals who are eligible for services. Key Responsibilities: Demonstrate awareness of a range of frameworks to assess and plan interventions. Facilitate the individual's management of their condition and treatment plans. Develop written documents relevant for practice. Able to build compassionate and effective relationships. Be able to communicate effectively. An ability to manage time, prioritise work and meet deadlines. Knowledge/Experience; Knowledge and understanding of relevant legislation affecting vulnerable adults. Knowledge of risk and safeguarding. Knowledge of relevant equipment and assistive technology. Knowledge and understanding of the principles of assessment / care planning and review.
