Service Support Worker - Assisted Discharge Location: Yeovil District Hospital, Somerset (requires travel within the area) Contract type: Fixed term until 31st of March 2021 Hours per week: 14 (Thursday & Friday, 11:00 - 19:00) Salary: GBP 15,682 per annum pro rata We've got an opportunity Do you have excellent people skills? If so, this could be your chance We have a fantastic opportunity for flexible and committed individuals to join our Independent Living team in Musgrove Park and Yeovil Hospital. You will provide reactive, focused and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to prevent hospital admission. You will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users' needs in order to re-enable service users to remain in their own home and prevent hospital re-admission. Referrals may require you to take the patient home from hospital to enable their discharge. You will also be providing practical and emotional support in the service user's own home and signposting to other services where appropriate to enable the individual to live independently in their own home. Also, you should be the following: Educated to GCSE level (or equivalent by experience) IT literate Full driving licence holder allowing to drive manual vehicles Have a good knowledge of services provided by the NHS and Social Care Please note that due to insurance requirements, all applicants must be aged 21 or over and have had their full driving licence for 2 years.
