Overview
Spencer Clarke Group are working closely alongside Dudley Council to provide them with several Assistant Care Coordinator’s to join their team on 3-month interim contract , likely to be extended. Contract – 3 Month agency contract with a view to being extended on an ongoing basis. Working Hours – Monday – Friday 9-5pm Rate of Pay – £11-£13 per hour – Negotiable depending on experience. About the role; As an assistant Care Coordinator you will be involved in the process of Care Management under the overall responsibility of Senior Care Co-ordinator/Team Leader. You will be working to a sensitive, responsive and appropriate response to individual service users and their carers. Please note we are in communication with all hiring managers and where possible remote working will be made available as well interviews via telephone. Key Responsibilities: To assist Senior Care Co-ordinator in the process of assessment for those service users who require specific assessments. Undertake assessments where needs are clearly defined and arrange appropriate Care Plans. Participate in the Adult Care Duty system under the overall supervision of Duty Senior and to refer onto alternative sources of advice or assistance. To participate in the process of implementation, monitoring and review, within departmental guidelines, of Care Plans and initiate appropriate action as required. Operate within Departmental stated Principles, Policies and Guidelines including Equal Opportunities, Health and Safety and the Complaints Procedure. Maintain records, undertake reports in accordance with departmental policy and practice.
