Overview
One of our Central London based clients is looking for a Social Worker to fill an available position in their Community Mental Health Team. The Qualified Social Worker will fill the role of Care Coordinator. This Social Worker role will involve working with vulnerable Adults who are suffering from a range of Mental Health problems. Great transport links via the Central Line. Responsibilities of the Social Worker: Carry a caseload of vulnerable Adults who suffer from a Mental illness. Putting into place care plans is accordance with the Care Act (2014). Mental Capacity Assessments. Practice Under the Mental Health Act (MHA) 1983 (2007). Working as part of an MDT to facilitate the best possible care for the service uses. Facilitate service user choice and empowerment. You must have: Social Work England registration. A clear enhanced DBS on the update service. At least 2-year post qualifying experience.
