Transition Work (SEND) with Forward Thinking Local Authority in North East London Reference: 119582 Type: Permanent Salary (£): Up to £39,774 depending on experience Job summary This innovative local authority is looking for an ambitious team player with experience in working with adults to join there SEND team. The successful candidate will be part of a multi-disciplinary team which uses a new operating model, which ensures social workers have access to all relevant information by combining the best functions of the Adults, Children, Public health and Housing services. Having received a Good Ofsted rating and continuously improving its services, this local authority works hard to deliver an exceptional experience for both service users and staff by adapting its practices to reflect new developments in the field of social sciences. Task and responsibilities: Helping young adults from the age of 18 transition from Children's Social Services to Adult Social Services by providing support where necessary.
