Description Being a key position in the company, we welcome applications from candidates with a Level 3 Health & Social Care Diploma as a minimum; with a desire to progress to a Level 5 qualification.Working 37 ½ hours per week, 9am to 5pm Mon-Fri (with a 30 min lunch break).We require flexibility in the role to meet the needs of the business, and the successful candidate will be required to be available on an ‘on call rota’ to cover weekends on a rotational basis. Duties/ Responsibilities Based in office premises in Brighouse and reporting directly to the Deputy Manager.The Care Co-ordinator is working alongside the field care supervisor; responsible for the team leaders based in the community and ensuring safe delivery of day-to-day care across the Southern Calderdale community. As the Care Co-ordinator you are responsible for scheduling and organising calls to clients, and supervising and supporting team leaders and the team of community support workers. Essential/ Desirable Skills · Level 3 Health & Social Care Diploma (minimum & Essential – with a desire to progress to level 5; supported and funded by the company). · Report Writing skills (Essential) · Assessment Skills (Essential) · Good understanding of CQC Regulations & standards (Essential) · IT – Competent in the use of an Electronic Management System (EMS) (Preferably People Planner) and MS Office proficient - (Essential) · Driving Licence and own car (Essential) · Knowledge of the Care Act 2014 (Essential) · Knowledge of GDPR (Desirable) · 2 years’ experience in a previous role (Desirable) · Previous transferable line management experience (Desirable) Essential Attributes · Excellent communication skills both written and verbal (Essential) · Strong organisational ability – able to prioritise, work under pressure and to meet deadlines · Self-motivated and reliable – flexible with a ‘can do’ attitude · Organised and methodical with good attention to detail · Trustworthy and discreet – to maintain the strictest of confidentiality at all times · Good values and ethics to fit in and meet with that of the Trailblazer’s Ethos; caring, kind, empathetic, compassionate, honest, respectful, able to work with discretion and confidentiality, show respect and honour the dignity of all Salary is £10.70 per hour, £20,865 per annum.
