Recruiting for 2 x Support Worker (4 month FTC) to join our Mental Health Service in Brent GBP 21,944 per annum, 40 hours per week plus 25 days Annual Leave, Pension schemes, Staff discounts, healthcare and more The service works with customers with severe and enduring mental heath conditions or learning disability who are assessed as having medium to low support needs.at Brent NAIL we have 2 services, one which is an all female accommodation, which has 4 female customers and 1 which is an all male accommodation based with 11 customers Most customers are referred to the service following a period in high support services. The aim is to offer support tailored to assist the individual developing skills to be able to live independently in the community. Customers are supported in a number of areas to enable them to 'step down' to lower support or independent living. Those include: maintaining a stable mental health, tenancy management, living skills such as cooking, budgeting, maintaining their room to good living standards, liaising with external professionals involved in their care, and linking with local governmental and third sector agencies to promote education, training, volunteering and employment opportunities. Key Responsibilities: - Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Contract Manager - Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions - Create support/action plans for providing appropriate services based on the assessment and maximising the use of resources available - Support customers to carry out domestic tasks, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation - Develop and maintain links with key agencies and service providers in the local community - Empower customers to ensure they receive the service and benefits they are entitled to - Undertake all administrative work and keep accurate and detailed tenant/customer records to high standards - Encourage and enable tenants to pay their rent and service charge ensuring that rent accounts are managed effectively - Ensure that referrals are sought and assessed proactively so that properties are filled as efficiently as possible to house those in need - Adhere to Look Ahead's Policies and Procedures - Engage in learning and development activity to increase knowledge and skills - Undertake any other duties consistent with the grade and nature of the post as may be reasonably required.
