Overview

CARE COORDINATOR ROLE PROFILE As a Care Coordinator within the Sugarman Health & wellbeing Division it is expected you will provide and coordinate individualised care and resources to service users living in their own homes and within the community. You will manage and distribute resources effectively and will ensure that all Domiciliary Care is provided in a respectful way; keeping in line with all relevant policy and procedure. With your excellent understanding of complex care, you will work closely with the multidisciplinary teams, such as Social Workers, District nurses, clinical commissioning groups and service user family members. You will ensure the effective management of these complex relationships and with the support of your branch manager will deliver the care and wellbeing service to the individuals being supported. It is essential that you provide exceptional customer service and operate in a manner consistent with company spirit, policy and procedure; working to deliver the business's long term vision and business goals. RESPONSIBILITIES You will ensure that there are sufficient workers available to meet client/service user requirements. You will manage and monitor complex shift rotas to ensure that the level of care provided supports the users needs. With the support of your Registered Manager & Team Leader, you will remain up to date with changes and updates in relevant legislation and Group Policy and Procedures You will monitor and communicate with multidisciplinary teams any changes relating to the health and wellbeing of the Service User. You will ensure that all workers registered are compliant with CQC essential standards and Group Policy and Procedure. You will be responsible for maintaining worker records in line with both Sugarman Health & Wellbeing operating procedures and group standards You will maintain Mobile Worker records in line with both Sugarman Health & Wellbeing operating procedures and group standards You will assist and resolve mobile work queries competently and comprehensively and handle and rectify client queries Mobile Worker Travel scheme and NEST Pension Scheme queries will be referred by you in line with Group standards. any matters that cannot be resolved by you will be escalated to your line manager or relevant support function. You will be responsible for communicating to the family and necessary government bodies, or any changes of condition or needs of the Service User. You will effectively manage the relationship between Service Users, Families and Government Bodies PERSONAL SPECIFICATION You will have the ability to work unsupervised whilst always maintaining an appearance that is suitable to represent the business. You will be able to effectively multitask, adequately prioritise a busy and demanding workload and work towards and within deadlines. You will possess excellent communication skills with the ability to work collaboratively and in the spirit of the Company and your position whilst having the ability to delegate where necessary. You will demonstrate continual development of your own knowledge, skills and capability within a complex care environment You will possess a sound knowledge of complex care, working with people who have complex care requirements. You will have the confidence and professionalism to attend multidisciplinary meetings, safeguarding meetings and deal with sensitive complaints.
