This is a great opportunity for a Social Worker to join the Hospital Team. This is a locum Social Work position based in South West of London. The Social Worker will be in a busy environment working to KPI's and reducing hosptial addmisions. Your responsibilities as a Social Worker Work to KPIS's in the Hospital Help reduce Hospital Admissions Carry out CHC (Continue Health Care) assessments Undertake Mental Capacity assessments Contribute to an effective practice for the service user Have clear communication with other practitioner involved with the service users Requirements of a Social Worker Qualified Social Worker BA/DipSW/CQSW or equivalent Current registration with Social Work England 2 years post qualifying UK experience Why should you choose Charles Hunter Associates?
