Overview
This is a great opportunity for a Social Worker to join the Hospital Discharge Team. This is a locum Social Work position based in South of London. The Social Worker will be working in busy environment working to KPI's. The Social Worker will be aiming to reduce Hospital admissions and carrying out after care plans and packages for Service users. Your responsibilities as a Social Worker Undertake a caseload involving the service users Work to KPI's Implement Care Act, Safeguarding and Mental Capacity assessments Undertake reviews Carry out Care plans Carry out care packages Requirements of a Social Worker Qualified Social Worker BA/DipSW/CQSW or equivalent Current registration with Social Work England 2 years post qualifying UK experience Why should you choose Charles Hunter Associates?
