COMMUNITY CARE TEAM LEADERS - GUARANTEED HOURS This is an exciting opportunity to play a role with a leading Health and Social Care Charity. You will take on the role of Community Care Team Leader working with the team to promote our values while contributing to improving lives of our service users. Community Care Team Leaders are responsible for mentoring care support workers, dealing with breakdowns in the community, starting new care packages, completing observations and supervisions in the community to ensure a safe, effective and quality caring service is delivered.
