Leaving Care Social Worker

Our client, Tameside MBC is looking for a registered Social Worker with Leaving Care / 16 experience. The responsibilities will include Care planning of young people 16 who are looked after and supporting them in respect of their independence skills in order to move on into independent living. You will also need to be confident in assessing the young persons ability to make the transition to independent living prior to them becoming 18. The Ideal candidate must also have experience of working with a multi discipline of professionals and be competent when case recording, completing regular care planning meetings and updating pathway plans. You'll need to consider a number of factors in respect of their needs which relate to their health, education, social/emotional, family relationships and contact and accommodation. Assessing their ability to make the transition to independent living prior to them becoming 18 and liaising with alternative agencies should they have vulnerabilities where it may not be suitable for them to live alone. You will be working with a multi discipline of professionals which involve CAMHS, Police, Catch 22, LAC Nurses, Schools/Colleges, accommodation providers and adult services where necessary. In addition to this I have regular contact with the young people on my caseload by undertaking six weekly statutory visits and having 8 weekly contact with care leavers. To apply for this role you must hold a degree, post graduate or Professional Qualification in Social Work. Hold full HCPC registration. Ideally have 3 years post graduate experience in the relevant team desirable Have Clear and valid DBS disclosure Have access to their own car.
