Overview

REED Socia Care Hub are currently recruiting for Level 1 Qualified Social Worker (Safeguarding Adults Team). This position is based in Sheffield. Duties will include: Continuing Health Care & Decision Support Tool to enable work for Hospital Discharge. Carrying out assessment in the service users home. CHC checklist and attending DST Setting up care packages-support plans Maintaining update electronic diary Apply knowledge and understanding of Policy and procedures for safeguarding adult Computer literate Multi-agency partnership working with Medical professionals, Occupational Therapist third party organisations and care providers £250 referral bonus - if you aren't interested, maybe you know someone who is?
