Overview

The function of the SEND and Vulnerable Children Caseworker is to provide high quality casework across 0-25 age phase that ensures that children and young people with SEND and those who are Looked After by the Local Authority achieve outcomes that are aspirational, inclusive, supportive of independence and designed to prepare for the next learning phase and ultimately for adulthood including independence and employment in their communities. To ensure all children with special educational needs, or children and young people looked after, are able to access the best possible education provision that ensures ongoing progress and achievement, good educational outcomes and are prepared for adulthood. The central responsibilities of the SEND, Looked After Children and Vulnerable Groups Caseworker is to: Facilitate the statutory assessment processes for children and young people Produce high quality Education, Health and Care Plans (EHCP) to support the identified special educational needs for individual children Monitor the education of Looked After Children Ensure identified groups of vulnerable children and young people have appropriate education and support in place to enable their progress and achievement The post is based at County Hall, Durham but the post holder will work from and across venues within the County Durham. Requirements: Degree in relevant field Enhanced DBS Experience working with 0-25 ages.
