Overview

The Hall Lane service in North Leeds provides Supported Living for adults who have a learning disability and complex needs, including autism, complex behavioural and communication needs. The service compromises of four individual properties supporting 15 individuals on one site. We are seeking experienced practitioners to undertake the interesting and responsible role of Support Co-ordinator for four individuals with a range of needs and aspirations. You must be confident, warm, highly motivated and committed to providing an excellent service. You will have previous relevant experience, knowledge and a good understanding of working with people with a learning disability who have complex needs. You must be skilled in assessment and support planning, person-centred planning and the use of positive behaviour support strategies. You will need to demonstrate a commitment to using person- centred thinking and approaches in your work to achieve great outcomes for the people we support. As well as providing direct support and leading the service at a designated house your role will include undertaking duty manager responsibilities across the service on a rota basis. You will manage and deploy staff effectively to meet the needs of service users, taking into account their personalised schedules and activity plans. You must be professional and able to act as a role model, leading your team by example to ‘go the extra mile’ to support service users. You will be patient, professional and well organised with excellent communication skills, both verbal and written. You will be able to liaise proactively and effectively with Care Managers and other members of the multi-disciplinary team. You will build warm, trusting relationships with service users and their families. An NVQ3/H&SC Diploma Level 3 or equivalent is desirable, but not essential; you will be willing to train to level 5 in the Diploma. A degree or other relevant professional qualification would be desirable. As this is a 24 hours/7 day a week supported living service you will need to be flexible and responsive to changing needs and demonstrate willingness to work the full range of shifts.
