Care coordinator Chenai Holistic Home Care, a provider of quality domiciliary care services, are seeking an experienced Care Coordinator to work in their main office in Dagenham. This is a full-time post that requires someone with Experience within the care industry is essential as well as knowledge of scheduling software. Responsibilities: Ensure that the Quality of Service Provision meets with policies and expectations. Care Coordinator ensures that Regulatory Requirements are met. To implement a high-quality service through effective matching of service user needs with suitably qualified and experienced care workers. Care Coordinator liaise with the registered manager to ensure that there is enough cover to deliver all visits and that new clients are planned into the rota in a timely fashion. Ensure working patterns are efficient and planned logically. Care Coordinator effectively prioritise tasks to meet unexpected emergencies and changes in care packages. Ability to maintain clear and accurate written records and follow statutory reporting procedures Care Coordinator ability to communicate clearly and build positive working relationships with clients, their families, fellow staff and other health and social care professionals. You could also be involved in the following: Receive and handle complaints about services from service users or others; Carry out on-call coordinating duties outside normal office hours as agreed. The ideal candidate will have: A strong background in Health and Social Care A minimum NVQ Level 3 qualification or above Strong IT and logistical skills Exceptional communication Organisational skills Good geographical knowledge We are offering: 40 Hours per week generous on call payments 28 days paid leave.
