Overview

The role will involve the following duties: 1. To assist in care coordination through assessment, support planning and review activity as required, ensuring risks have been identified and addressed in order to provide a safe personalised service to meet specific needs which fall within the agreed eligibility criteria. 2. To support service users to remain in the community by means of practical and emotional support as part of an agreed package of care. 3. To respond appropriately and in a timely manner to customer enquiries and provide accurate information to members of the public. 4. To undertake enquiries under the Safeguarding Adults policy and procedure where agreed and appropriate and any investigations or referrals commensurate with the role 5. To keep updated information about available resources from a wide remit of health and social care agencies and other organisations. This will enable teams to provide high quality information and advice to customers. 6. To ensure that service users and their families have access to translation and interpretation services as appropriate to their cultural and religious needs. 7. To record information required to monitor and evaluate the effectiveness of the service. This will ensure that outcomes will be measured in relation to the speed and type of response that will support people in the community, facilitate hospital discharges and promote independence. 8. To be responsible for recording details on the electronic system, ensuring all records are accurate and up to date for collection of statistical data. To comply with relevant legislation and the Data Protection Act. 9. To undertake additional or amended duties appropriate to the post as may be necessary. Specific Features of Social Work Assistant role in the Older Persons Mental health team: • To ensure on-going work, care plans and care packages are reviewed in accordance with Departmental policies, contractual agreements and in liaison with appropriate colleagues, partner agencies and service providers. • Determine any issues relating to a client’s circumstances (including family and carer arrangements) to ensure that the relevant parties are involved and that the circumstances are reflected in conjunction with a client’s right to choice and control over how their needs and capabilities are reflected in their assessments and reviews.
