Overview

We’re looking for ANP with Advanced assessment and preferably Non-medical Independent Prescriber skills (please see attached JD’s for details). Our service operates 24/7, the successful candidate will be expected to cover 37.5 hrs per week (LD 8am – 8pm and N shift 8pm – 8am). It’s community based rapid response team covering Ealing. Job Summary : keeping the people who using our services as safe as possible through the use of sound clinical skills and effective risk assessments Ensuring best possible clinical outcomes by using up-to-date skills and adhering to evidence based practice and relevant policies and procedures Ensure the people using our services have a good experience by respecting, empowering and working in partnership with people throughout the care planning process Provide line management and clinical supervision to an agreed cohort of staff Undertake initial assessments and interventions to patients acting as first contact Contribute to the success of the team by being an effective role model and supervising other Band 5 nurses, healthcare assistants/ support workers and students Provide advice and support (physical, guidance and supervision) to the Intermediate acre team and wider Ealing Community Partners (ECP) team. Act as first point of contact for referrers and health care professionals Person Specifications: Qualifications and Training: Registered AHP or RG/MN Teaching Qualification Advanced Assessment (lvl 7) NMP (lvl 7) Accredited Advanced Practitioner Experience Experience of reviewing deteriorating patients Experience of providing patient facing interventions Able to interpret results both biochemistry and near side Experience of leading on recruitment Working with a Nursing, Physiotherapy and Occupational therapy team within an inpatient and or community setting Experience of providing clinical information to wider MDT Experience of working within a community or in-patient setting as undergraduate Experience of leading on QSIR/quality improvement projects Experience of training and providing supervision Experience of providing clinical interventions within an acute and community setting Experience of developing career pathways Knowledge Knowledge of KSF and where this role functions Knowledge of patient safety measures Knowledge of safeguarding Working knowledge of NHS 10 year plan Knowledge of frailty and impact on wider health determinants Understanding of admission avoidance Personal Qualities Able to work seamlessly with other professional groups Communicate clear message and guidance Remain professional at all times in line with NHS KSF
