Overview

We are currently looking to recruit an experienced Children's Support Team Leader in the Wakefield area. At Yorhealth we provide nurse-led care for children and young adults with complex medical care needs throughout Yorkshire. We are CQC registered and fully insured to provide care in family homes and thier local community. We operate 24 hours a day, 7 days per week. Care is overseen by a registered sick children’s nurse and supported by trained health care support workers. Job Summary The Children's Support Team Leader will provide outstanding care for children and young people with complex needs in their own homes. Working in partnership with the family, the role involves the provision of personal care, tube feeding, medication administration, seizure management, posture and airway management, emotional care and play under the delegation of the registered nurse. This role involves management of care planning and care records. Ongoing training and development is a key feature of this role. Main Duties and Responsibilities 1.1 Carrying out the day to day cares for the child / young person which includes provision of personal care, pump gastronomy feeds, medications administration, gastronomy care, seizure management, airway management, emotional care and play. You will accompany the child/ young person on trips out of the home eg, school and hospital and holidays. 1.2 You will contribute to a team approach to the care and support of the family working with all members of the multidisciplinary team . 1.3 Taking responsibility for documentation and evaluating care planning, you will ensure the legality and accuracy of the child’s care plan/ and or other reports and the effectiveness of the established communication /reporting system over the full 24hrs . 1.4 Participate in regular meetings and attend other meetings as requested . 1.5 Undertake tasks and procedures, in which competency has been assessed and recorded specific to the care environment, including for example, airway suctioning and pump feeding . 1.6 Participate in research activities as and when required. 1.7 All team members are responsible for behaving in a manner that does not put themselves, children and young people, families, other team members or others in danger or at risk in the care environment 1.8 To assist the family in the maintenance of stock levels and management of equipment where required 1.9 To assist in the cleanliness and tidiness of the home care environment and adhere to infection control policy and procedures at all times 1.10 To assist the child/young person to ensure their individual dietary requirements are met 1.11 To receive and convey information in a courteous manner and families are assisted in a welcoming and supportive manner 1.12 To assist and support the team in administrative duties as required eg time sheets, rota planning, shadowing and supporting new members of the team with shadowing etc Becoming a Children's Support Team Leader requires you to work flexibly, this will include working evenings, weekend and sleep in/waking night duties as required. Yorhealth Care offer a continuous service, 365 days per year.
