Job Title: Social Worker - East and North Herts Older People's Service, Integrated Hospital Discharge Service Location: Based in Farnham House, Stevenage Salary Range: H8-M1 £26,999 - £38,813pa Hours: 37 hours per week Contract: Permanent Job Duties: An exciting opportunity has arisen in our Intermediate Care and Out of County Hospital Team, this Service supports an integrated approach to managing care of people who require support on discharge from hospital. Your team will be based in Community Rehab Units, Care Homes and office based. This team is based within our Integrated Hospital Discharge Service working closely with the Acute Trusts. To improve outcomes for service users, the ICT and Out of County Hospitals Team support a 7 day service. The successful candidate will therefore be asked to work on consecutive Saturdays and Sundays once every eight weeks, and will receive days off during the weeks before and after covering a weekend. The successful candidate will be based in Farnham house, Stevenage but will be required to travel to other sites across the County. You will need to be HCPC registered and have knowledge and experience of relevant legislation. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.
