Overview

Service Coordinator Location: Worthing, Crawley and covering West Sussex region Salary: £21k Full time permanent vacancy working 37.5 hours per week which will include an 'on call’ rota. We have a superb opportunity for an enthusiastic, organised and motivated individual to join our team covering the West Sussex region, as a Service Coordinator. You will be working closely with the management team to provide support and practical assistance to ensure a quality driven and effective service is provided to all users. Your role will include; To undertake and review needs assessments, risk assessments, support plans and person- centred plans for each person that we support. To record and communicate to others the wishes and needs of the people we support. To assist with staff rosters, ensuring the rota staffing levels meet the needs of the people we support. To assist the Registered manager with recruitment and selection of support workers. To direct support workers in their duties and undertake supervision meetings, appraisals, performance review and observations including the care certificate. To monitor and promote each persons’ health and wellbeing. To assist the Registered manager with audits and quality assurance throughout the service. Liaise with other agencies, medical professionals, family and carers. Contribute to the review and development of strategies in response to challenging behaviours. Provide on call support and assistance when required. Skills, experience and qualifications required: NVQ Level 2 and 3 in care Previous experience within a professional care environment. Driving licence and own transport would be beneficial but not essential. Enthusiastic and caring individual who is passionate about service delivery. Knowledge of CQC standards, health & safety and quality assurance. Ability to work under pressure to meet deadlines. A positive, energetic attitude, keen to develop new ideas. Excellent communication skills and a team player. A flexible approach to working hours.
