Reablement Social Worker, Darlington Council

Reed Social Care are currently working with Darlington Council to find multiple Social Workers with prior experience in Adult's Services to join the authority's Reablement Services The aim of this service is to undertake strength-based assessments with the aim of reducing, delaying or preventing the need for long term support Some duties for this service are: - Completion of strength-based assessments and reviews - Initiating safeguarding enquiries - Facilitating discharge from hospitals - Signposting to relevant teams and services - Completing Mental Capacity Assessments - Compiling and reviewing support plans Applicants must have a qualification in Social Work to degree level or equivalent (e.g BSc, BA, MA, CQSW, DipSW) and be registered with the Health and Care Professions Council (HCPC).
