My Homecare currently seeking experienced Care Coordinator to join our team Own transport is an essential requirement of this role, full clean UK driving licence Roles & responsibilities As a Care Coordinator you will lead, develop and coordinate a team of care workers to provide the highest standards of respect and care, ensuring the dignity of the clients is maintained at all times. You need to be a good team leader, communicator, listener, to be focused, dedicated and reliable. Your job position would consist of both Community Care and part Office-Based duties Benefits offered. Pension (Nest Pension scheme) High standard of Training Paid Holiday Permanent Contract Your responsibilities will include: · Conducting assessments with new service users and regular reviews with existing clients · Preparing support plans, care plans and all required documents for service users’ folders and ensuring that all files are maintained accurate · Building relationship and corresponding with hospital teams, social workers, doctors, nurses etc · Preparing the weekly rota · Training senior and care workers, allocating and supervising their work and development · On-call duties · Covering calls and on-call if and when needed · Carrying out regular spot checks and supervisions · Assisting in holding staff meetings · Carrying out risk assessments under the Health and Safety requirements. · Assisting with satisfaction surveys, monitoring staff performance, training and supervisions · Maintaining confidentiality and observe data protection and associated guidelines where appropriate. . Completing the responsibilities of Community Support Workers while providing care in community. Hours of Work: As Care Coordinator, you will be contracted to 40 hours per week, with a minimum of 20 hours per week of office duties to complete your Care Coordinator responsibilities. This may vary week to week. Weekends will be included when needed.
