Job Description
Worthing Responsive Services and Therapy Hub is a multidisciplinary team providing a seven day service. We provide: * A rapid response to people in the community who are experiencing acute episodes of ill-health * A supported discharge service to facilitate timely hospital discharges * Short term support for people who require assistance with activities of daily living * A rehabilitation service including a range of therapies * Community rehab for people with long term conditions and frailty We are looking for Band 6 or experienced Band 5 Physiotherapists to join the therapy team. The main focus for these posts will be assessing, treating and monitoring patients with long term conditions and frailty, as well as providing rehab within their own homes. There is strong multidisciplinary working with the Local Community Network (LCN), and the post holder will jointly manage, with other therapists, the collective caseload for their specified geographic area. The post holder will need to have experience of working within a variety of clinical settings, particularly the community. Competence in lone working is essential. Car driver and car owner are essential due to the geographic area covered by the team. There are full time and part time posts available which allows the post holder to have greater flexibility in working hours.
