Job Description
Are you looking to develop your skills and knowledge as a Senior Nurse in the community? An opportunity has arisen within the Responsive Services team in Brighton and Hove for a highly skilled, motivated and enthusiastic individual to join our team. Based at Bicentennial building in Chichester, we deliver services across the Chichester, Midhurst, Pulborough and Selsey areas, and within patients' normal places of residence. As a Senior Nurse you will join our multidisciplinary team which consists of Nurses, Occupational Therapists,Physiotherapists, Therapy Assistants, and Generic Support Workers. Working collaboratively with our acute Trust and other community services including third sector organisations, we aim to improve the patient journey and pathways of care and deliver short term intervention, care and support in the community. Responsive Services delivers the 'right care at the right time in the right place', playing a key role in supporting patients' discharge from hospital and community beds and supporting patients to stay at home, preventing avoidable admissions to hospital.
