Job Description
The West DIST team is comprised of experienced staff from a variety of disciplines. Our staff are compassionate, very proactive and highly skilled at managing risk and supporting older people with organic / functional illnesses and who present in crisis in the area of West Norfolk. We aim for all our service users and carers to receive care appropriate to their individual needs in the least restrictive environment. The team is the 'front-door' to acute services, they provide triage, assessment, intensive support and admission The staffing teams also works with service users already in hospital to ensure that they can be supported at home at the earliest opportunity to support their recovery. The West Norfolk dementia and intensive support team DIST team were rated good in the 2019 CQC inspection. We want you if you are: We are looking for a band 5 registered nurse who is keen to learn, develop and be part of transformation and improvement in the area of delivering intensive support in the community We want creativity, innovation and passion towards providing excellent patient care; someone with a level head who is aware of and able to cope with the pressure and unpredictability of acute intensive support care. We are looking for someone who can communicate and advocate in a professional manner with a wide spectrum of people to achieve the best outcome for our service users and their families /carers. The team works 11.30 hr shifts over a 7 days week service to cover 08:00hrs - 20:00hrs However most importantly and in keeping with our trust vision and values of Positively, Respectfully and Together, we are looking for someone who has exceptional people skills as this role requires you to make difficult decisions whilst having to maintain professional relationships.
