Buckinghamshire Healthcare NHS Trust is delighted to announce the opportunity for a Community Practitioner Physiotherapist working within our Rapid Response & Intermediate Care Team based in Amersham. Full time hours are available but applications for part-time hours will be considered. Are you a registered Physiotherapist with a broad range of clinical experience? About us Rapid Response and Intermediate Care (RRIC) is a team of Physiotherapists, Occupational Therapists, Intermediate Care Nurses, Community Healthcare Assistant Practitioners and Healthcare Assistants. The Amersham team is one of seven teams across the county. We provide specialist assessment and rehabilitation in order to facilitate timely discharges from hospital and prevent unnecessary hospital admissions. We work with patients to empower and enable their recovery from illness or injury, maximise their independence and provide support to those at the end of life. Our clinicians provide holistic comprehensive assessments, expert clinical interventions and set up appropriate rehabilitation plans for support staff to follow. We pride ourselves on the high quality, compassionate care our team provides across the county. We promote an integrated approach and work with colleagues in community nursing, primary care, hospital and social care to support people in their own homes. The Trust is committed to supporting the education of Allied Health Professionals and there are opportunities for staff to progress their careers with our leadership programmes. About you You should have a broad range of clinical expertise, ideally with experience of working in a community multidisciplinary team environment within the NHS. Experience of managing healthcare assistants or other support staff is desirable. However, we would provide support and training as required and there is the potential to further develop your clinical expertise in a range of areas. As an expert clinical resource for the team you would be expected to support staff to develop their knowledge and skills. You must be innovative in your approach and willing to work with other members of the community teams to ensure high quality service delivery. Additional information In order to respond to the healthcare needs of the local population, our community teams operate 7 days a week ensuring the delivery of a service which can react to the patients' needs as and when required. The shifts for RRIC Community Practitioners are generally 08:00 - 16:00 including weekend and bank holiday working as part of a rota. Currently RRIC Community Practitioners work one weekend per month and approximately one weekday late shift (10:00 - 18:00) per week. Weekend and Bank Holiday working is subject to enhanced pay in accordance with Agenda for Change Terms and Conditions.
