Fixed term contract until February 2021 Are you passionate about ending rough sleeping, and supporting people to rebuild their lives? About the role We have an exciting opportunity to join the well-established, dynamic Reading outreach team in the Pathway Navigator role. In the role of Pathway Navigator you will work alongside the Reading outreach team working with an agreed cohort of people sleeping rough in these areas, supporting individuals to make the move away from the streets. You will also have the opportunity to provide tenancy sustainment support, ensuring people who have previously experienced rough sleeping in these areas maintain tenancies to avoid a return to homelessness. You will be managing a Complex Case worker to support the transition from the streets or from supported accommodation into an independent tenancy for clients You will be required to build and maintain productive relationships with internal and external stakeholders including Local Authorities, DWP, other homeless services and housing providers. Working in the team office and in the community, you will provide a client centred approach, offering support and advice to access health care for physical and/or mental health conditions, work and training opportunities and the relevant support for other support needs. The role will require you to provide imaginative routes off the streets, and work in collaboration to achieve positive results. Core hours are 9 - 5, Monday - Friday, however, flexibility is required as the post-holder will be expected to work some bank holidays. What we're looking for This role will suit someone with experience of engaging vulnerable people with complex/multiple needs and the ability to work with challenging behaviour. You will also have an understanding of the issues faced by homeless or vulnerably housed people and the difficulties they experience in accessing services. A knowledge of housing legislation and welfare benefits; and the ability to manage your own workload while working independently in a fast faced environment is also essential.
