If you are a caring, empathetic and flexible individual and able to provide expert care and support then you could now join our community mental health team and colleagues on a fixed term contract basis. (2 x 37.5hrs - Duration up to 12 months. 1 x 30 hrs - Duration up to 6 months). Our customers can't wait to meet you when you apply. If you have experience of providing both individualised and customer-led care whilst also promoting wellbeing and support you will know working in this field of support is highly rewarding. You will be helping to improve lives through * Maximising the independence of people who require support and enabling real choices in how they lead their lives * Providing housing related support such as supporting people to access their benefits entitlements; pay their rent and household bills; secure appropriate accommodation; access employment, volunteering and training opportunities. * On occasion assisting some people with personal care, administering medication and undertaking domestic chores. * Maintaining accurate records of support provided, maintaining risk assessment and support plans We value you...
