An exciting opportunity has arisen for a self motivated, experienced Diabetes Specialist Nurse (DSN) to be at the forefront of driving up standards in diabetes care within the Community Diabetes Team for Lincolnshire Community Health Services NHS Trust. To be appointed as a Band 7, the successful applicant must have extensive experience of working autonomously within the field of diabetes and be a non medical prescriber with the ability to work to evidence based guidelines; audit the care given to patients to make improvements in services. The post holder will also be required to deliver educational packages to health care professionals and patients around effective diabetes management. Consideration would also be made for an experienced Band 6 clinician who wishes to develop their diabetes skills and knowledge to be become a future senior specialist in diabetes nursing. You will also have excellent interpersonal and communication skills, be highly motivated and positive in your approach to care delivery to ensure we maintain a high quality service delivery to our patient population. You must be able to travel across the County to locations not served by public transport. Be Part of Our Team...
