Job Description Main duties of the role include: General Responsibilities Maintaining regular communication with line manager and office staff; Liaising with other health and social care professionals and agencies to identify those in need and at risk within the local community; Undertaking and writing up assessments to company standards; Arranging for further counselling or assistance in the form of financial or material help; Organising support and developing care plans to address service users' needs; Keeping case records and data, preparing reports and participating in team meetings; Participating in training and supervision. Client Care Interviewing individuals and groups to assess and review the nature and extent of difficulties; Contributing to a positive living environment to enhance a client's quality of life; Documenting daily activities and reporting any changes or causes for concern in a client's needs or living conditions is essential Stimulating and encouraging through a variety of activities (for example clothing selection, hobbies and exercise); Accompanying to appointments, providing assistance with mobility and transportation; Running errands and performing light housekeeping tasks; Assisting with organising medication plans and providing medication reminders; Assisting with bathing, dressing and grooming; Assisting with continence care; Providing advice on dietary guidelines, meals planning, assistance with preparation of meals as well as assisting during mealtimes; Providing Dementia / Alzheimer and other neurological impairments care Manipulating and massaging patient to discover the cause of pain, relieve discomfort, restore function and mobility and to correct irregularities in body structure; Referring patients who require further medical or surgical attention Assessing and providing treatment for people with mental disabilities, or those suffering with mental illness, stress, and emotional and relationship problems. Requirements for the role: Basic Requirements: Ability to treat and care for clients and their property with dignity and respect; Ability to adapt to various living environments and locations; Ability to listen, communicate clearly and build relationships with clients and their families in a friendly and pleasant manner; Willingness to work and communicate professionally with other social and health care professionals; Good hygiene practice, including personal hygiene and a smart appearance; Good level of stamina and fitness to meet the physical demands of the job; Good time management skills; Friendly and approachable attitude; High level of personal responsibility; Driving Licence.
