We have an exciting opportunity for you to join a first class community healthcare service! Sussex Community NHS Foundation Trust (SCFT) are looking for motivated and enthusiastic Band 5 Registered Nurses to join our amazing teams within our two inpatient wards at Bognor Regis War Memorial Hospital. We specialise in short term rehabilitation for patients over the age of 18. Our aim is to prevent unnecessary acute hospital admissions and to support timely discharges from the local acute hospitals. As one of our Band 5 Registered Nurses you will lead a small team of Healthcare Assistants and together you will contribute to the care of our patients through observing, monitoring and recording their progress during their rehabilitation with us. This role is essential in communicating and effectively managing any changes in our patients' conditions, right through to planning their safe discharge. Our Nurses holistically assess, plan, implement and evaluate patient care, whilst working closely with the multidisciplinary team to ensure patients reach their rehabilitation potential. We work on a seven day shift system with occasional night duty by arrangement. We are very happy to discuss working patterns with you as well as support your transition from an acute or non NHS site.
