Crisis and Home Treatment Nurse Practitioner, Band 6 Bromley Home Treatment team have an exciting opportunity for a Band 6 Mental health Practitioner to join our well established team which has extended its hours of operation to provide a 24 hour service. The team provide a comprehensive service to meet the needs of the residents in the Borough of Bromley. Come and join a successful and dynamic service based at Green Parks House , Bromley providing an effective alternative to inpatient treatment for people in mental health crisis, 365 days a year. The Home Treatment Team is an essential component of the Crisis Care Concordat 2014 and forms part of comprehensive borough-based crisis services, alongside the Mental Health Liaison, Inpatient care. The team is multi-disciplinary and employs a number of nursing, medical, Occupational Therapists and support staff who work closely to provide a viable alternative to hospital admission and a skilled resource to support early discharge. The team also gate-keeps all referrals for inpatient care, carries out assessments of people referred by GPs for urgent assessment, and Joint working with Community mental health teams. The post is suitable for a Registered mental health Practitioner with recent experience in an acute adult inpatient, crisis or community mental health setting. You should enjoy working within a busy and challenging environment, and must be able to work effectively under pressure. You will have a sound knowledge of mental illness and therapeutic interventions and competence in mental health assessment. You will be creative and innovative in your approach, and will work collaboratively with patients and their carers in the formulation of care and crisis plans. Your communication skills will be well developed and you will be able to effectively liaise with other mental health teams, the local authority and emergency services. The successful candidate will receive a robust induction to prepare them for this stimulating role. The team is friendly and enjoy good working relationships with other services.
