Job Description

An opportunity has arisen for a Band 6 Community Clinical Lead Nurse (Adult Learning Disabilities) within the Darlington Adult Learning Disabilities Community team. The team is based at Hundens Lane Resource Centre, Darlington and covers the Darlington locality. The successful candidate will need to have a genuine desire to improve the mental and physical health and well-being of adults with a learning disability. They will work as part of an established multi-disciplinary team, the successful individual will be responsible for managing their own caseload and delivering high quality person centred care. They will be expected to work closely with other disciplines within the team and the wider multi-disciplinary team (including external agencies) to provide evidence based nursing assessments and interventions based upon individual needs. The team core hours are 9am to 5pm, with flexibility for planned out of hours work to meet service user needs. Staff are also included in an nurse on call rota (approximately once per month ) Applicants must possess a good clinical knowledge base of the health needs of people with learning disabilities and have experience of working with this client group. The post holder will be expected to work with the multi-disciplinary team; however they may work independently under supervision of senior team staff. The successful applicant will be responsible for the supervision of junior staff and will also be expected to support students on placement with the team. They will possess good interpersonal skills and be committed to providing a high standard of care for people with learning disabilities. Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.
