Job Description
Community Nurse CLCH Community Planned Services are currently seeking a motivated band 5 nurse to join our community based service in Barnet. Are you looking for opportunities to develop your community nursing skills in your career and to be part of a unique and dynamic integrated care team? Do you want to be part of developing and shaping a new integrated care service in Barnet? If so, an exciting new opportunity is available for a Registered Nurse to join our newly restructured Planned Integrated Care Service. Barnet Planned/Unplanned service provide community Nursing services to Barnet CCG and is split into three locality teams. This post will be located in one of the Barnet localities. The post holder will work closely with the team leads/ clinicians to develop the service and maintain close working relationships with both acute and community services. We are offering the exceptional candidate the opportunity to rotate internal though our newly conducted planned and unplanned services. This will give you experience in the fast track Rapid response/PACE unplanned team, and the community nursing team. Employment Type: FULL_TIME
