About Us Warwickshire County Council's Mental Health Services are integrated with Coventry and Warwickshire Partnership Trust (CWPT) under a long-standing and successful Section 75 agreement. The integrated and multi-disciplinary teams have been organised into Integrated Practice Units (IPUs) which allow for person-centred care and support to be tailored according to service users' and carers' needs. The IPUs are based on defined national care clusters: Non Psychosis (clusters 3-8), Psychosis (clusters 10-17) and Organic (clusters 18-21) and all work with people who have complex and enduring mental health problems requiring care co-ordination under the Care Programme Approach. Each IPU provides outcome-focused interventions which also includes Care Act assessment and support-planning to address social care needs. This Lead Practitioner (LP) post is one of several, each aligned to an IPU, and has been created in order to provide leadership and supervision to our integrated social care staff as well as promoting the social care agenda within the IPUs. This specific post is aligned to the 18-21 Dementia IPU and although it is a countywide post the successful applicant will spend the majority of their time working with our staff in the Rugby and Nuneaton bases. The Role Ideally we're looking for an Approved Mental Health Professional (AMHP) who can additionally provide a small contribution to the daytime AMHP rota. But more importantly we want a passionate and enthusiastic leader who has experience of developing and motivating those they work with. Someone who can work autonomously, who is creative and imaginative when it comes to meeting service users' needs, and someone who cares deeply about their work and what they can bring to a team. The successful candidate will work flexibly to manage a challenging workload. You will provide professional supervision to several social care staff who work within the CWPT IPUs, and who are line managed by a CWPT Team Manager. This means you working alongside, and in partnership with, Health colleagues and consistently promoting the social care agenda throughout. As well as supervision, you will be evaluating and approving social care funding. You will take a lead on Safeguarding cases and must have experience of promoting and protecting service users' right. You will also investigate complaints and at times may deputise for the operations manager. You will also need a good knowledge of legislation such as the Care Act, the Mental Capacity Act and the Mental Health Act.
