Job Description
Our community hospitals aim to provide comprehensive care to local people aged over 18 with the with the aim of preventing unnecessary hospital admissions, supporting timely discharges from the acute hospitals, and providing palliative care. Patients are admitted from the community or from surrounding local acute Trusts after stabilising their acute medical needs. Kleinwort Centre, Haywards Heath - 6 Programmed Activities per week You would be working in our 23-bed unit in Haywards Heath, supported by our Advanced Nurse Practitioner. Geriatricians in-reach weekly to support cross system working. You will be the medical leader in the team, providing expert primary and community care for patients who require inpatient care but are not unwell enough to need admission to an acute hospital. You will have a proven clinical background with an interest in quality improvement and service innovation. This post will allow you to combine your primary care knowledge with your passion and experience for more complex medical condition management in a non-acute clinical setting. Candidates will also have opportunities to develop medical and clinical leadership skills on Trust-wide and health system-wide projects. There is regular support from Consultant Geriatricians and the SCFT Clinical Lead as well as study leave time, dedicated SPA time and opportunities to attend medical education meetings. Appointment would be to the Specialty Doctor, Associate Specialist or Salaried GP grade depending on experience and qualifications. You must have NHS experience.
