Job Description
an exciting opportunity to join NHS Tayside Perth and Kinross HSCP has recruitment opportunities for healthcare professionals to develop an enhanced community service for patients who experience long term respiratory conditions. With the support of services that are integral to respiratory care, the team will develop and provide an evidence based community service. This will include working with local general practices to support and manage long term respiratory patients who are housebound. The team will be responsible for monitoring and managing patients to prevent further deterioration and complications of their condition, by promoting self management, education and support to maximise the quality of life of patients. Future development is envisaged to provide a pulmonary rehabilitation service that supports patients post hospital discharge. The service will consist this lead practitioner role, with an overview of service development, team management and a clinical workload and 2 further clinical practitioners. You should be a First Level Registered Nurse or Registered Allied Health Professional with evidence of continuing professional development and appropriate knowledge and qualifications, for example diploma level knowledge for asthma and COPD and education in smoking cessation. Extensive post registration experience is required. You will be involved in collaborative working, therefore, strong leadership, interpersonal and communication skills are essential. Ability to travel across Perth & Kinross is essential.
