JOB DESCRIPTION In this role as the Care Coordinator you will effectively and efficiently manage the delivery of high quality care services, by taking sole responsibility of scheduling staff rotas, monitoring evaluating the carers use of the company's software Mobizio system to ensure high efficiency and quality delivery of services to our customers. Responsibilities * Ensure that Care Worker rotas are organised correctly matched to Clients' needs and well-planned, taking into account travelling time, holiday cover and last minute cancellations. * You will be in charge of making sure all visits are carried out. Where there are cancellations you will liaise with Field Supervisor to find cover however where no cover is available at short notice, you will cover the calls ensuring high stands and reliability to our service users. * You will participate in the on call schedule including weekends as arranged * You will schedule rotas weekly in a timely fashion on Mobizio software ensuring accuracy and making sure carers have logged unto the system to access their rotas in advance. You will also communicate all the rotas to the Field Supervisor to ensure all staff are aware of the weeks' schedule and that all visits are accurately allocated. * You will monitor all care calls/ visits on Mobizio ensuring carers are logging on, off before, and after each call. * You will ensure their assigned tasks are being checked off as per their duties for each assigned clients. You will be in charge and responsible for flagging any calls and tasks not logged and ensuring you notify the carer to complete off tasks or se the system appropriately. * You will provide help and support to the care staff on the field where they are experiencing issues with the software that is unable to log on or needing assistance to use their device correctly. * You will flag any reported incidents or prompts from the carer's or Field supervisor regarding a service user immediately to the attention of the Registered Manager to resolve without any lapses in time or communication. You will also record this on the Mobizio record of the client where necessary. * Responsible for resourcing for new care packages and building on existing packages. * Improving and maintaining the continuity of care for new and existing care packages. * Maintain clear, accurate, appropriate and up-to-date digital records for all Clients and Care Workers using our bespoke system. * Responsible for cordially resolving customer telephone queries in a timely manner and professional fashion to maintain a positive Company reputation. You will notify the Registered Manager as appropriate. * You may be sometimes be required to undertake initial Client assessments in Haringey and North London and to put together detailed Care Plans, thorough risks assessments using the company's Mobizio software. Please note you will need authorisation from the Department Head, Rhoda before undertaking this task. * You will monitor and evaluate Carers Spots checks, appraisals, training and flag up to the Registered Manager when trainings are due for renewal. You will also prompt the Field Supervisor to ensure timely Spot checks and Supervisions with the carers are carried out in a timely fashion. * You will monitor the results of the spot checks and supervisions and feedback to the Registered Manager where further training is required or additional action is required. * Ensure all operations are Company and CQC Compliant. * Contribute to the growth and service expansion targets set by the business. * You will maintain confidentiality at all times and carry out the Company's Confidentiality Policy. * Ad hoc tasks as required ad requested REQUIREMENTS * Ideally NVQ/QCF level 3 in Care * Very organised, methodical, and a good eye for detail and accuracy. * At least 2 years' experience working as a care coordinator for a Domiciliary Care Agency. Live-in care co-ordination experience is preferred. * Knowledge of CQC Compliance is essential. * Ideally experienced in conducting care assessments, care reviews and being able to motivate care staff. * Being flexible around working schedules, roles and responsibility as it is a fast growing start up environment * Being a team player is extremely crucial to this role * Professional and confidential at all times
