Job Description
We are looking to recruit a Qualified Social Worker in the Continuing Health Care Team within Hackney. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels. The responsibilities of the Social Worker includes: * Demonstrate experience in developing good working relationships and partnership working with other departments and services within London Borough of Hackney, City & Hackney CCG, Homerton Hospital, and Integrated Independence Team. * Able to challenge CHC decisions constructively and positively. * Work with an in-depth knowledge of both legal and statutory frameworks relating to social work practice. * Undertaking a small caseload of complex CHC assessments and some OOB Hospital referrals, you will also undertake joint visits and review with IIT therapists for those on the IIT D2A Home pathway. * Ensure care and support are delivered in a cost efficient manner, identifying networks, local resources, assistive technology, equipment and adaptations etc To ideal candidate will have: * Social Work England registration * Eligibility to work in the UK * Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA) Working with Ackerman Pierce you will be guaranteed weekly payment and access to continued profession development, including a dedicated consultant to support you through the interview and on boarding process.
