Overview

Role summary To provide clinical advice and practical support to social care colleagues and people who use the service in order to increase knowledge, skills and competence, assist in quality standards being met and ensure key areas of need are met. Act as a source of advice and expertise in relation to clinical aspects of Learning Disability / Autism and ensure health needs are met. Role specific Carry out nursing and clinical assessments Participate in the assessment of new referrals including the formulation of costed support package submissions Actively contribute to the development and implementation of care and support plans including advise and provide direct support in relation to support plans that relate to the clinical aspects of Learning Disability and Autism including those associated with communication, skills development, advocacy, decision making, autonomy and risk Ensure that a continual focus is maintained on individual’s health needs including contributing to a health action plan Work in collaboration with PBS Practitioners, PBA Assistants, Team Managers and other staff to ensure effective therapeutic interventions in relation to complex and challenging behaviour Actively participate in the management of critical incidents and crisis management when required Liaise with external health and associated professionals including community teams in relation to individuals who use the service in order to ensure a collaborative approach and clear communication especially in relation to health needs and risk management Participate in staff training and development including delivering training sessions, coaching and mentoring staff when required Ensure high standards of service delivery including full compliance with organisational, legal and regulatory requirements Report any concerns, complaints, significant incidents immediately Network with stakeholders, other providers and key professionals to actively promote the development of the service and enhance its reputation Participate in business development and associated networking activities Actively promote a culture of a person focused service, openness and honestly General Safeguard customers, their families and any vulnerable adults or children that come into contact with services against potential or actual abuse or harm Take all reasonable precautions to maintain the health and safety of self and others, ensuring that any risks in relation to this are reported and remedial action taken in a timely manner Comply with the services policies and procedures Actively support and promote the services objectives, operating model and core values Take responsibility for own continuous professional development and maintaining an adequate level of knowledge and skills pertaining to role Undertake any other duties that may reasonably be required by the service This Job Description is neither exhaustive or exclusive and may be subject to change in light of service requirements and development Person spec Essential Desirable Current NMC registration Diploma or Degree in Nursing RNLD qualification PBS training Articulate, good communication skills Other relevant clinical training Flexibility regarding work location – driver PROACT-SCIPr-UK training Self-starter – able to work on own initiative Professional presentation and approach
