Overview
Sutton are looking for a BIA adults assessor role Previous experience of completing BIA assessments (including capacity assessments) - stating how many completed. Copy of their qualification and Social Care England Registration number. Proof of insurance cover A completed form 3 DoLS assessment anonymised and if they have not got a capacity assessment as part of that form 3, a separate capacity assessment to evidence practice please. Rate £200 per assessment plus mileage where travel is over ten miles from Civic Centre.
