Occupational Therapist

An excellent opportunity is available for an Occupational Therapist , to join an outstanding Community team in Gravesend and Sheppey. The job role is to work as a specialised Community Occupational Therapist within the Community Therapies Team, responding to referrals from a variety of sources, across the Gravesend and Sheppey areas. A valid driver's license is essential for this role. The successful post holder will be responsible for their own caseload, and continuously providing a high standard of care and Occupational Therapy services. They will work with patients who have diverse and often extremely complex conditions, and it's therefore essential to have postgraduate experience in physical health . Aspects of the full role will include: Assessing, planning and delivering patient centred care Preventing Hospital Admissions Facilitating timely discharge Maximising function and quality of life Essential Requirements: Diploma/Degree in Occupational Therapy HCPC registration Experience of student and junior supervision Ability to independently travel , to manage a community caseload (Gravesend & Sheppey areas) This post is advertised as Band 5-6 , and the salary offered will be dependent on your skills and experience.
