Social Worker - Multi-Disciplinary Team - Blackburn - £25.02 Per Hour Blackburn welcome a passionate and dedicated individual to join them in their multi-disciplinary team. You will enjoy working alongside adults to provide short intervention, goal setting and positive outcomes for people through a Discharge to Assess Pathway which supports patient journey out of hospital, back to their own home and connect them back into their communities through our neighbourhood model. We are looking for passion, drive and motivation with the right behaviours, core values & attributes. Excellent communication is essential as is an ability to prioritise and manage your own caseload, having good organisational skills, resilience and flexibility.
