Overview
Social Worker - Hospital -Merton Full-Time, Substantial post qualifying experience, HCPC Registered, Diploma in Social Work or equivalent Degree. Job Description As a Social Worker responsible for working effectively with service users to achieve positive change in outcomes for the individual while in hospital. The role is based within the council's hospital, the team supports service users aged 18 and over, who have a variety of needs and across all specialisms. You will need to be able to complete assessments, statutory reviews, mental capacity assessments, safeguarding, carers and continuing health care assessments. The hospital team can receive a number of Section 2 referrals and is subject to the Delayed Discharge Reimbursement Act 2005. The team also receives referrals of safeguards - these will often involve working alongside the police and health partners. Team members are supported to complete safeguarding investigations, continuing health care assessments and undertake other roles as valued members of a multi-disciplinary team. As part of a hospital assessment team you will work in partnership with their health colleagues to promote independence and help service users take control of their own lives. About the client Our client based in the Midlands is one of the leading services for Hospital Discharge. The authority implemented a 'quick discharge service'; this is a 24/7 support service for citizens being discharged from hospital and is part of the new pathway for homecare from hospital. Job summary Visiting and interviewing clients to make needs assessments relating to timely hospital discharges Undertaking FACE assessments Assessing priorities and writing reports to develop care plans. Accepting responsibility for actions in relation to the liberty or safety of clients in emergency situations. Including carers and clients' families in the assessment process in line with the Equal Opportunities Act 2004. Liaising with colleagues and other agencies as appropriate including domiciliary, day and residential services. A hospital social work team which is focused on arranging safe and timely discharges for service users Supervision of the set-up of Community Alarm Systems Onward referrals to CAB (citizen advice bureau), Income Maximisation Service, Community Rehabilitation Teams Participation in Multi-disciplinary team meetings and ward meetings Attendance at home visits sometimes jointly with Occupational Therapists Liaison with GP surgeries, district nurses, rehabilitation facilities and other professionals Liaison with housing associations and/or the council to facilitate rehousing to sheltered accommodation when necessary What we need from you Degree or equivalent in social work Current HCPC registration Significant senior level frontline experience within children's services 2 years post qualified with experience within local authority child protection teams If you are passionate about delivering a high standard of social care and would like to work in a supportive forward-thinking environment, we would like to hear from you if you think you meet the requirements.
