Qualified Social Worker Hospital Discharge Team Your new organsiation Qualified Social Worker job recently registered, working for the hospital post discharge team based in their South Birmingham location. The office is a citywide post-acute hospital intervention service based at Lifford House, with social workers covering both adults and children Your new role The role requires you to manage a workload which will include some vulnerable adult service users, some with complex needs, carry out assessments and formulate support plans. You will be working in partnership with service users, families and local community groups as well as with Health, Housing, Education, voluntary groups and wider public services. What you'll need to succeed Previous direct experience of delivering social work to Adult Service users is essential. You must also have knowledge of key legislation, able to produce reports and maintain records. A recognised social work qualification is a necessity for this role.
