Overview

Home Support Worker - 20 Hours Per Week Birmingham Visiting recently discharged patients in their homes is essential therefore you will need a drivers license and use of car. (Business Mileage claimed) The care provided by a Home Support Assistant is that which gives the people who use our services the choice of living independently in his or her own home, and does not include tasks which would normally be undertaken by a trained nurse. The following list illustrates some of the duties, which a Home Support Assistant may be asked to undertake. To work within a network of services delivering a multi-disciplinary care plan, consisting of nurses, social workers, occupational and physiotherapist and health and social care co-ordinators and support staff, to promote and enhance the independence of the people who use our services. To visit people who use our services in their own homes and other community settings, for examples: Extra Care Accommodation and other similar settings to provide the appropriate level of support as defined in the Care Plan. This will include aspects of health related personal care tasks, hygiene, exercises, activity and treatment programmes prescribed by therapists. The aim being to rehabilitate and maximise the person who is using our services, potential and where appropriate to support the carer. As part of the care given to people who use our services, it will also be necessary to undertake administration and clerical tasks that are associated with the care process. To support and enable people who use our services in a range of activities to promote independent living and monitor their wellbeing: o Personal care/bathing/showering/toileting/shopping o Meal preparation o Supervising and/or administering medication o Confidence building o Exercise programmes/mobility o Basic wound dressings - for example; a me pore dressing Requirements: Experience of working with adults with a disability or older people, older people with mental health needs. Ability to work shifts, evenings, weekends and Bank Holidays. Must be willing to travel throughout the Borough. Legal Requirements: Candidates must be 18 years or over. Enhanced Criminal Record Bureau Check and Protection of Vulnerable Adults check. Genuine Occupational Requirement Must hold a valid full current drivers licence.
