Overview

Tradewind Social Care are currently recruiting for an experienced Adult Social Worker to join a Transitions Learning Disabilities team in North London. The main purpose of this role is to be responsible for assessments and care planning for children, young people and young adults with disabilities and preparing them for the transition to adulthood Main Duties: To carry out needs led assessments of children, young people and young adults within the guidelines of Children's Act 1989 and 2004 and Community Care Act To care manage both straight forward and complex cases including Safeguarding Children, Young People and Adults and Deprivation of Liberty and mental capacity Co-ordinating and reviewing multi-service care packages To formulate appropriate care plans and negotiate care packages with service users and service providers To apply eligibility criteria to determine eligibility for support and other children's statutory eligibility criteria as appropriate To provide support plans to service users in receipt of services from the department To update care plans following reviews, and negotiate care packages with service providers To maintain appropriate and accurate up to date service user's records and to provide statistical information and reports as and when required To co-operate in research projects where appropriate To contribute to the development, planning and implementation of policy and practice relating to the care management of service users and carers To facilitate and foster effective relationships between local authority, NHS, private and sector partners and service providers and to represent the service at meetings or relevant panels, chairing and minuting when required, in order to facilitate the effective provision of services to users To prepare young disabled people for transition to adulthood To have responsibility for a portfolio of cases for a defined area of work as determined by the line manager Participate in the rota duty system at all times with other members of the service unit and to respond to emergency situations when required Essential Skills & experience needed: Good communication skills; listening, understanding, ability to express self to people from a range of backgrounds and cultures and ability to negotiate with service users, relatives and providers Will be expected to work in a conflict environment and within situations that they may find challenging Ability to make sound professional judgments Ability to provide excellent verbal and written reports which can be considered within high level and legal settings Be able to make critical decisions on behalf of the Authority which may be in conflict with the wishes of the service users and/or their families or care providers Ability to complete Care Act, mental capacity and best interest assessments, and also safeguarding investigations Experience in working with older adults with learning disabilities Experience using Liquid Logic IT system would be advantageous Job Requirements: An enhanced DBS that is of satisfactory level is essential Must have a minimum of 2 years experience All applicants need to be social work qualified with current HCPC registration Our promise to you: A highly experienced recruitment consultant who will provide you with regular updates about vacancies across the UK Career guidance alongside our free CV writing service Free yearly DBS disclosures Prompt and reliable payroll service Excellent aftercare service This job was originally posted as www.totaljobs.com/job/89933339
