Overview
Community Frailty Nurse Type: Full-time Band: Band 6 Hours: 37.5hrs We are a friendly, dynamic PCN in Poole. We are looking to expand our multi-disciplinary team and appoint an experience Band 6 nurse to work in our Integrated Care Team and to provide a visiting service for our house-bound patients. The ideal Candidate will provide a holistic approach to assessment, diagnosis and treatment to acutely presenting patients. You will: Be clinically competent in all aspects of primary care Be able to work independently and perform home visits Have excellent clinical and communication skills Be able to demonstrate critical thinking in the clinical decision-making process A UK driving license and their own vehicle A positive attitude towards General Practice and life in general Liaise closely with senior GPs and other clinicians Have superb written and verbal communication skills We offer competitive rates of pay, NHS pension scheme, a superb working environment and a setting where career development and progression is encouraged and supported.
