Overview

We are looking for a Band 4 Support Worker for a fantastic opportunity to join a Specialist NHS Community Severe Mental Illness (SMI) Service travelling between various South East Essex services (Rayleigh, Canvey Island and Southend-on-Sea). •Do you know someone who would be interested in this role? Temporary pay rate: £14ph ltd Hours and length of contract: •Full time Monday to Friday 09:00 - 17:00. •The manager is looking for the position to be filled ASAP with an initial 3 month contract length. Daily Duties: You will be working alongside the Community Mental Health Services (CMHT) and GP Surgeries to undertake both physical and mental health assessments of the service users experiencing serious mental illness. You will help enable the service users to access the support they need to address their physical and mental health needs and promote recovery. Duties will include but are not limited to: •Assessing both physical and mental health needs of service users and ensuring needs are met •Undertaking annual physical health checks of patients who have been identified as having severe mental illness - blood pressure, pulse, weight, BMI, venepuncture for required blood tests. •Holistic assessments of service users lifestyles - exercise and activities, eating and drinking, smoking, alcohol and drug use and giving advice on healthy lifestyles and signposting patients to relevant services. •Working alongside the physical health nurse in wellbeing clinics and community mental health teams - team working skills are essential •Completing relevant records in relation to physical health checks and interventions •Maintaining electronic records in an accurate and timely manner.
